What does the Transition Coordinator do?
The Transition Coordinator can fill many roles, depending on the needs of the individual and the team. Some of the things we do include:
- Attend schools meetings on a regular basis, including IEP meetings and all other meetings relating to a students transition plan.
- Act as a resource to the team gathering needed information and community resources.
- Ensure DVR involvement and funding post-graduation.
- Complete all necessary CIP funding/Dane County Human Services paperwork.
- Receive individual's vocational rate from DCHS. Advocate for increases in the rate, if needed.
- Communicate with Vocational Providers about their services, ability to take on new referrals, and provider rates.
- Assist families with interviewing and choosing a Vocational Provider.
- Assist families with interviewing and hiring a Support Broker.
How do you get the names of students exiting?
By August of the upcoming school year, we receive a list of all graduates in Dane County from Dane County Human Services Intake Unit. They obtain this information from the schools.
When does the Transition Coordinator first make contact with schools? Families?
We initially make contact with families and school personnel in August and early September. At this time, we discuss the Dane County Transition Policy and our role on the team. We also find out when the first school meeting is.
Do you talk about Self-Directed Services (SDS)?
The Dane County Intake Unit discusses SDS at the first eligibility meeting with students and families. We discuss SDS throughout the year. After a vocational rate has been established by Dane County, we assist students with interviewing and hiring a vocational agency to support them post-graduation. Towards the end of the school year, we assist students and their families with hiring a Support Broker who will coordinate future Self-Directed Services for that individual.
Is there a form the Transition Coordinator prepare for each exiting student?
The Transition Coordinator completes all necessary CIP paperwork prior to graduation, including an Individual Support Plan (ISP) and an Individual Financial Plan (IFP). If a DVR application has not been completed, we can also assist with the application process. We also complete a Vocational Profile that is shared with vocational agencies after being hired by a student and his/her family. This form outlines a student's vocational support needs, work history, work schedule, etc...

SYSTEMS TRANSITION COORDINATOR
Planning:
The Systems Transition Coordinator identifies a group of developmentally disabled students aged 18-20 years to "pair" vocational settings. This requires communication with all transition teachers in MMSD (Madison Metropolitan School District) as well as all POS vocational providers.
Parent Transition Group:
The Systems Transition Coordinator will develop and sustain a Parent Transition Group to assist families in preparing for the transition to adult services. The group will consist of students with disabilities aged 18-20 years, their parents and teachers. The group will meet at least quarterly. The meetings will be a mix of trainings and feedback sessions.
