Job Title: Systems Transition Coordinator
Reports to: Executive Director
Minimum Education/Experience Requirement: A degree in the Human Service field or the equivalent and at least two years experience with adults with developmental disabilities is necessary for this position. One year Support Broker experience is desirable.
Purpose: To assist high school students aged 18-20 years with developmental and other disabilities and their families prepare for transition to the adult developmental disabilities system.

The Systems Transition Coordinator is expected to identify a group of developmentally disabled students aged 18-20 years to pair their vocational settings. This will require communication with all transition teachers in MMSD (Madison Metropolitan School District) as well as all POS vocational providers.
Transition Coordinators will attend all PCS staff meetings. Information is given between meetings that the Transition Coordinator is responsible for with additional information and brainstorming sessions occurring at the staff meeting.
The Transition Coordinators are expected to complete all mandatory Support Broker training offered by the Support Broker Coalition and Dane County. Transition Coordinators may be required to attend information sessions regarding the support of their clients throughout the year as a means of gaining resources while also input to the transition of graduates.
The Systems Transition Coordinator will develop and sustain a Parent Transition Group to assist families in preparing for the transition to adult services. The group will consist of students with disabilities aged 18-20 years, their parents and teachers. The group will meet at least quarterly. The meetings will be a mix of trainings and feedback sessions.
The Systems Transition Coordinator works closely with DCHS personnel. Regular meetings are scheduled and will be attended by the Transition Coordinators. The Executive Director is involved in many of the meetings and can become involved in individual situations as necessary. The Systems Transition Coordinator will attend the Supported Employment Provider Coalition and Transition Teachers meetings as necessary in order share information and gather feedback on the transition process.
Transition Coordinators are required to report any suspicion of abuse and neglect to the Executive Director. Refer to the PCS Abuse and Neglect Policy for Support Brokers.
The Systems Transition Coordinator is responsible for case noting all interactions as per protocol.
Transition Coordinators meet annually with the Executive Director to establish personal goals for the year.
