Job Title: Transition Coordinator
Reports to: Executive Director
Minimum Education/Experience Requirement: A degree in the Human Service field or the equivalent and at least two years experience with adults with developmental disabilities is necessary for this position. One year Support Broker experience is desirable.
Purpose: To assist high school graduates with developmental and other disabilities and their families with the transition during the last year of high school to the Dane County adult system.

The Transition Coordinator is expected to become a part of the graduate's team. As part of the team, the Transition Coordinator is required to disseminate information regarding DCHS policies regarding graduates and the person's IEP. The Transition Coordinator will develop a plan of support for each graduate and assist the graduate and his/her family with interviewing agencies and support brokers near the end of the school year.
All Transition Coordinators will attend all PCS staff meetings. Information is given between meetings that the Transition Coordinator is responsible for with additional information and brainstorming sessions occurring at the staff meeting
The Transition Coordinators are expected to complete all mandatory Support Broker training offered by the Support Broker Coalition and Dane County. Transition Coordinators may be required to attend information sessions regarding the support of their clients throughout the year as a means of gaining resources while also input to the transition of graduates.
The Transition Coordinator works closely with DCHS personnel. Regular meetings are scheduled to discuss each graduate and his/her progress on his/her plan during the last year of school. The Executive Director is involved in many of the meetings and can become involved in individual situations as necessary. The Transition Coordinator will be asked to attend the Supported Employment Coalition meeting as necessary in order share information and gather feedback on the transition process. The Transition Coordinator historically requests information in the form of a survey from agencies in order to develop a spreadsheet for the Transition Coordinators, DCHS and ultimately the graduates to utilize when choosing agencies.
The Transition Coordinator must be aware of the State and Federal funding programs, and how these relate to personal and county funding. Transition Coordinators negotiate with support providers while advocating with Dane County when necessary.
All Transition Coordinators are required to report any suspicion of abuse and neglect to the Executive Director. Refer to the PCS Abuse and Neglect Policy for Support Brokers.
The Transition Coordinator is responsible for a variety of paperwork for both DCHS and State of Wisconsin funding. The Transition Coordinator is expected to complete all SDS paperwork along with a CIP application for each graduate prior to each graduate's receiving adult services from DCHS. All interactions for a graduate will be case noted as per protocol.
Each Transition Coordinator will meet annually with the Executive Director to establish goals for the year.
