2009 Vocational Transition Planning
In 2009, we expect to transition 50 or more graduates from Dane County high schools into long term adult services, a 50% increase over previous years. The logistics and economics of transitioning such a large group will be a significant challenge in an already stretched fiscal environment. To achieve the successful transition of all graduates in 2009, Dane County plans to directly assign the graduates to vocational support agencies. We believe the direct referral of grads to supported employment agencies will ensure that all grads receive needed services. Individuals will interview and hire their support broker/case manager with the help of the county’s Self-Directed Services (SDS) Coordinator. These interviews typically take place in early June. We hope that all graduates will be satisfied with the services they receive from their provider. If concerns arise after transition and a change is sought, individuals will have the ability to give notice and change agencies, within the limits of their vocational support rate.
